The form requests your name, phone number, email address, preferred date and time, and preferred contact method. The site may also use an approximate connection country to show information for U.S. visitors and local or session storage to remember language and notice preferences. We do not request diagnoses, symptoms, X-rays, insurance information or other medical details through this form.

We use the information to respond to, manage and confirm appointment requests; communicate about availability; operate and protect the website; and maintain administrative records related to the request.
Providers and transfers
For these purposes we may use website hosting providers and Google Workspace for email and a private calendar. These providers may process information outside Mexico under their terms and safeguards. We do not sell personal information or use it for third-party advertising.
